Patient intake software for clinics
Send new and returning patients one link before their appointment. They fill the intake and anamnesis form, give informed and GDPR consent, upload an ID, and acknowledge the treatment plan, all on their phone. You start the appointment with the file complete instead of a clipboard in the waiting room.
- Patients complete intake before they arrive
- Informed consent and GDPR consent for health data, done digitally
- The file is ready when the appointment starts
What does the client actually do?
The patient opens the link you send by text or email, usually a day or two before the appointment. A guided form walks them through the intake and anamnesis questions in plain language, one at a time, on their phone. They give informed consent for the visit and GDPR consent for handling health data, upload a photo ID if your clinic requires one, and acknowledge the treatment plan when there is one to sign. It takes a few minutes, at home, instead of a clipboard in the waiting room.
What do you get back?
Structured intake answers land in one place before the patient walks in, not a stack of handwritten forms to file afterwards. The consent form and any treatment plan acknowledgement come back signed and sealed with an evidence page. Delivery is by webhook, REST or email into your practice system. Health data collected this way is encrypted, and how long you keep it is a setting on the workflow itself, not a fixed default.
Which documents does this replace?
The paperwork a dental, medical, veterinary, optician or physiotherapy practice hands a patient at the front desk moves online instead, filled from the same answers the patient already gave when booking. Nothing here is medical advice or a diagnosis; it is the intake and consent paperwork a visit already requires, collected before the patient sits down, on their own phone, without a printed form to lose or misfile.

Do I need the compliance steps for a clinic?
No. Clinics are not an AML-obliged sector, so there is no sanctions or PEP screening step to add here. What matters for a clinic is handling health data correctly: informed consent, a separate GDPR consent for special-category health data, and control over how long records are kept. Braigest encrypts patient data and lets you set a retention mode per workflow, from a full record to metadata-only, so a clinic's data handling matches what it actually needs to keep.
How does the PDF I already use become the form?
Upload the intake or anamnesis form your clinic already hands out on paper, in whatever language it is written. Braigest detects every field and turns it into a guided form the patient fills on their phone before the visit. The questions stay the ones your clinicians already rely on; only the way patients answer them changes, from a clipboard to a link sent ahead of the appointment.
Which plan fits a clinic of my size?
A solo practitioner, such as a single dentist or vet, runs intake and consent on Solo, at €29 a month. A small practice with a few clinicians and more appointment volume fits Practice, at €149 a month, which adds the AI builder for drafting new intake flows. A multi-location clinic group fits Firm, at €499 a month. The intake, consent and delivery mechanics are identical across tiers.
Frequently asked questions
Can patients complete intake on their phone before the appointment?
Yes. The link works on a phone, and patients typically finish the intake and consent form at home before they arrive, so the file is ready when the visit starts.
Is patient health data handled securely?
Yes. Data is encrypted and isolated per organization, and you set a retention mode per workflow, from a full record to metadata-only, so you keep only what your clinic actually needs.
Is the signed consent form legally valid?
It carries an advanced electronic signature with a PAdES seal and an evidence page recording consent, time and device. That is valid for private-law consent documents under eIDAS. It does not reach the top eIDAS signature tier, which needs a certified signing device.
Do clinics need identity verification or AML screening?
No. Clinics are not an AML-obliged sector, so there is no screening step built in for this use case. An ID capture step is available if your clinic wants one on file, but it is optional.
Can I use the intake form we already hand patients?
Yes. Upload the PDF and Braigest detects its fields automatically, so the same questions become a form patients fill on their phone before the appointment, without redesigning your intake paperwork.
